I was recently reading an essay on Holography in Japan by Mitsuo Kodera, in the Leonardo issue 'Archives of Holography', Vol 25 No.5, 1992, which contains a short history of Japanese Holography. He mentions that, in 1975, "Toppan Printing produced an embossed hologram for the English magazine Penrose Annual", which turns out to be a long-running survey of the printing industry and actually a substantial hardback book.
I located a copy and found, tipped in before the section of Developments in Japanese Printing, an embossed 2D diffraction pattern of a butterfly, making The Penrose Annual Vol 68 1975 probably the first publication to contain an embossed hologram – unless you know otherwise...
